Support Coordination
Support Coordination is all about upgrading your skills and being a part of your community.
A Support Coordinator will review the participant's NDIS plan to understand their goals, funded supports, and budget.
Explain the plan to the participant and their family or caregivers, ensuring they understand how to use it effectively and put it into action.
They can also help with :
Connecting with Services
Assist participants in finding and connecting with suitable service providers.
Research and recommend appropriate services, including therapy, community participation, and support services.
Building Capacity
Help participants develop the skills and confidence to manage their support and navigate the NDIS system independently.
Provide education on how to coordinate and utilize support.
Coordination of Supports
Coordinate a range of informal, mainstream, and funded supports to help the participant achieve their goals.
Ensure services are delivered per the participant's plan and monitor their effectiveness.
Crisis Management
Support participants in times of crisis or when facing significant challenges, ensuring they have access to necessary support.
Develop strategies to manage and mitigate future crises.
Plan Review and Reporting
Assist participants in preparing for their plan review meetings.
Provide necessary reports and documentation to support plan reviews and changes.
Advocacy
Advocate on behalf of participants to ensure their needs and preferences are heard and respected by service providers and the NDIS.
Budget Management
Help participants manage their NDIS budget to ensure funds are used effectively and sustainably throughout the plan duration.
